Kate Pedak Permanent Makeup

Please read these Terms & Conditions carefully before booking any service. By booking an appointment, you agree to the terms below.

1. Age Requirement

Clients must be at least 18 years old to receive permanent makeup services. Valid government-issued ID may be required.

2. Booking & Deposits

  • A non-refundable deposit of $30 is required to secure your appointment.
  • The deposit is applied toward the total cost of your service.
  • Appointments are not confirmed until the deposit is received.
  • The remaining balance is due on the day of your appointment.

3. Cancellation & Rescheduling

We value your time and ask that you respect ours.

  • Please provide at least 48 hours’ notice to cancel or reschedule.
  • Cancellations or reschedules with less than 48 hours’ notice result in forfeiture of the deposit.
  • No-shows automatically forfeit the deposit.
  • Arriving more than 15 minutes late may require rescheduling, and the deposit may be forfeited.

4. Medical Eligibility & Contraindications

Permanent makeup is a form of cosmetic tattooing and is not suitable for everyone.

You are not eligible if you:

  • Are pregnant or nursing
  • Have a history of keloids or hypertrophic scarring
  • Are currently using Accutane or have used it within the past 12 months
  • Are using Retinol or Retin-A near the treatment area (must discontinue at least 7 days prior)
  • Have an active skin infection, irritation, or open wound near the treatment area
  • Are undergoing chemotherapy or radiation (doctor’s clearance required)
  • Have a bleeding disorder or are taking blood-thinning medications
  • Have diabetes (doctor’s clearance required)
  • Have had Botox in the treatment area within the past 2 weeks
  • Have had dermal fillers in the treatment area within the past 4 weeks

All medical conditions, allergies, and medications must be disclosed during consultation. Failure to disclose relevant information releases us from liability.

We reserve the right to refuse service if a procedure is deemed unsuitable.

5. Informed Consent

Before your procedure, you will be required to sign a consent form confirming that:

  • You understand permanent makeup involves depositing pigment into the skin
  • Results vary based on individual factors and aftercare compliance
  • The procedure carries inherent risks, including infection, allergic reaction, scarring, asymmetry, or unsatisfactory color retention
  • You have disclosed your medical history accurately and completely

6. Results & Expectations

  • Permanent makeup is semi-permanent and will fade over time.
  • Healed results depend on skin type, lifestyle, sun exposure, and aftercare.
  • Specific results or color retention cannot be guaranteed.
  • Additional sessions may be required to achieve desired outcomes.
  • Following all pre-care and after-care instructions is essential.

7. Touch-Up Policy

  • Initial touch-up pricing applies only if scheduled within 30–45 days of the original procedure (exact timeframe will be communicated at your appointment).
  • Touch-ups scheduled beyond this period are considered a new service or annual refresher and are priced accordingly.
  • Annual color refreshers are recommended to maintain results.

8. Allergic Reactions & Complications

  • Allergic reactions to pigments or numbing agents, while rare, are possible.
  • A patch test is available upon request and recommended for clients with sensitive skin or known allergies.
  • We are not liable for complications resulting from failure to follow aftercare instructions or undisclosed medical conditions.

9. Refund Policy

  • All deposits are non-refundable.
  • Once a procedure has been performed, no refunds will be issued.
  • If you are dissatisfied with your results, please contact us to discuss available touch-up options.

10. Photos & Portfolio

  • With your consent, photos may be taken for documentation and portfolio purposes.
  • Images may be used on our website or social media.
  • You may opt out of photo usage at any time by notifying us.

11. Pricing & Payments

  • Prices are subject to change without notice.
  • The price confirmed at booking will be honored.
  • Accepted payment methods: Cash, Zelle, Venmo, and major credit cards.

12. Liability Waiver

By booking and receiving services, you acknowledge that:

  • You have read and understood these Terms & Conditions
  • You accept the inherent risks associated with permanent makeup
  • You release Kate Pedak Permanent Makeup from liability, except in cases of gross negligence

13. Changes to Terms

These Terms & Conditions may be updated at any time. The current version will always be available on our website.
